Stroke?

Tissue Plasminogen Activator (TPA)

Intravenous (IV) TPA:
- Administer within 4.5 hours of symptom onset for eligible patients.
- Screen for contraindications like recent surgery, bleeding disorders, or severe hypertension (>185/110 mmHg).
Intra-arterial (IA) TPA:
- Administered directly at the clot site for large vessel occlusions when mechanical thrombectomy is unavailable or contraindicated.
Eligibility Criteria:
- Early recognition of stroke symptoms and quick imaging (CT or MRI) are essential for TPA eligibility.
Device-Based Treatments

Why It Matters: Advanced devices offer targeted solutions for patients who cannot tolerate medications or have specific anatomical challenges.
AHA Guidelines:
Watchman and Amulet Devices:
- Recommended for patients with atrial fibrillation at high risk of stroke but unable to take long-term anticoagulants.
- These devices close the left atrial appendage to prevent clot formation.
Carotid Stenting and Surgery:
- For patients with significant carotid artery narrowing (≥70%), carotid endarterectomy (CEA) is the first-line recommendation.
- Carotid artery stenting (CAS) is an alternative for patients at high surgical risk or with unfavorable anatomy for endarterectomy.
- Both procedures aim to restore blood flow and reduce stroke risk in patients with carotid artery disease.
Mechanical Thrombectomy:
- For large vessel occlusions, mechanical thrombectomy within 24 hours of symptom onset is highly effective. It can be used with or without IV TPA.
Additional Considerations:
- All interventions require precise imaging to identify suitable candidates and optimize outcomes.

- Physical Therapy: Focuses on improving mobility, strength, and balance.

- Speech Therapy: Addresses communication and swallowing difficulties.
Psychological Support:
- Managing depression and cognitive changes is critical for recovery.
Antiplatelet Therapy

Why It Matters: Antiplatelet medications reduce the risk of blood clots forming by preventing platelets from sticking together. They are essential for both prevention and post-stroke management.
AHA Guidelines:
- Primary Prevention: Low-dose aspirin (75–100 mg daily) is recommended for select high-risk individuals, such as those with diabetes or significant atherosclerotic risk, after evaluating bleeding risks.
- Secondary Prevention: Antiplatelet agents like aspirin, clopidogrel, or aspirin-dipyridamole combinations are used to prevent recurrent ischemic strokes.
- Dual Antiplatelet Therapy (DAPT): In high-risk cases, such as recent minor stroke or TIA, short-term DAPT (aspirin plus clopidogrel for 21–90 days) is recommended. Long-term DAPT should generally be avoided due to bleeding risks.
Example Medications: Aspirin, Clopidogrel (Plavix), Aspirin-Dipyridamole (Aggrenox).
Anticoagulation Therapy

Why It Matters: Anticoagulants prevent clots by targeting clotting factors in the blood. They are especially important for conditions like atrial fibrillation, which significantly increases stroke risk.
AHA Guidelines:
- For patients with atrial fibrillation and high stroke risk (CHA₂DS₂-VASc score ≥2), anticoagulation therapy is strongly recommended.
- Options include warfarin (target INR 2.0–3.0) or direct oral anticoagulants (DOACs) like apixaban, rivaroxaban, dabigatran, and edoxaban.
- In patients with mechanical heart valves, warfarin remains the standard.
Key Considerations: Anticoagulants should be avoided immediately following hemorrhagic strokes but may be restarted later under careful monitoring.
